-Electronically sign the Consent for Disclosure of Medical Information under Forms
-Physical Examination (performed within 1 year for returning student-athletes and within 6 months of start of academic year for new/transfer student-athletes) Physicals can be uploaded under Forms
Additional Information:
-If you currently take prescription medication for ADHD, the ADHD Prescription Medication Documentation Form must be completed by a medical professional (Form can be found on main Athlete Portal page on SportsWare) then uploaded under the Forms section.
-If you choose to get a Sickle Cell Test performed, the Sickle Cell Trait Test Documentation Form must be completed by a medical professional (Form can be found on main Athlete Portal page on SportsWare) then uploaded under the Forms section.
-The Mount Ida Physical Examination Form MUST be used by new/transfer student-athletes. Returning student-athletes may use any documentation stating clearance from a medical professional (Form can be found on main Athlete Portal page on SportsWare). Physicals can be uploaded to SportsWare under the Forms section.
